How the Medicare & IRMAA Numbers Work
Your tier comes from income. Social Security sets your 2026 surcharge from the MAGI on your 2024 tax return. Enter that income and your filing status and the calculator finds your IRMAA tier — from no surcharge at all up to the highest bracket.
The cliff is the whole game. Tiers are all-or-nothing at the threshold. Cross a boundary by a single dollar and your entire Part B premium jumps to the next tier. The amber card shows how many dollars of headroom you have before the next step — and what crossing it would cost you per year.
Your true cost. The total adds Part A (usually $0), your IRMAA-adjusted Part B premium, your Part D plan premium, and the flat Part D IRMAA add-on — then shows it as a monthly figure, an annual figure, and a percentage of your income.
